Instructions for written review

1. Click here:    
2. Click on the red button "Write a Review".
3. If you have an account, sign in using your username and password (right side of screen) and skip instructions to step 9.
4. If you do NOT have an account, click on "Create an Account Now" on the right side of the screen.
5. Don't worry...you can use your current email address (you don't have to create a new one).
6. Enter the requested information.
7. Once accepted, an email will be sent to you verifying your request to join Google Maps. Even though you have not verified your email address yet, you can still write a review.
8. At the bottom of the Account Creation Confimation page, click on "Click here to continue".
9. Click on the "Stars".
10. A new screen may appear called Google Hotspot (if this is your 1st review).
11. Enter a public nickname that will be visible to others and click on "Start Rating".
12. Google will then take you to a new page and aske you to enter a category.
13. Type "Creative Smiles" or "Dr. Rick Kariotoglou".
14. Click on "Creative Smiles" (in blue writing) and it will bring you to Dr. Rick's Google Homepage.
15. Scroll down to "Rate & Review" and click on it.
16. Click on how many stars you think we deserve and write your review. Please leave first name, last initial so we can recognize you and enter your name for the prize.
17. Click on "Publish".
18. That all!! We appreciate your rave reveiw!!

Video Review

1. If you have a webcam or iphone, go ahead and record a 30-90 second video.
2. Send it to office@creativesmilesaz.com.
3. Once we get it published to Google, we will enter your name for an IPAD!
4. If you do not have a webcam, you are more than welcome to come to our office and record a video review here!
